 Waiver & Liability Release 

• Acknowledgment and Assumption of Risk 

I understand that participation in cheerleading, tumbling, stunting, dance, and related activities involves inherent risks, including but not limited to falls, collisions, overexertion, and equipment failure, which can result in serious injury or even death. I knowingly and voluntarily assume all such risks, both known and unknown, even if arising from the negligence of the released parties described below. 

I accept full responsibility for my participation and/or my child's participation in all activities, practices, classes, open gym, camps, competitions, and related events at or sponsored by Rising Stars Athletics. 

• Release and Waiver of Liability 

In consideration of being allowed to participate at Rising Stars Athletics, I, on behalf of myself and my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue Rising Stars Athletics, its owners, directors, coaches, employees, volunteers, and agents (the "Released Parties") from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury, including death, that may occur while participating in or attending any gym-related activity. 

This release includes any claims arising from the negligence of the Released Parties. 

Medical Authorization 

In the event of an injury or medical emergency, I authorize Rising Stars Athletics staff or representatives to obtain medical treatment for myself or my child, as deemed necessary by qualified medical personnel. I agree to assume full responsibility for any resulting medical expenses. 

• Media Release 

I grant permission for Rising Stars Athletics to use photos, videos, or other media recordings of myself/my child taken during classes, performances, or events for promotional purposes, including social media, websites, and printed materials.  

Our goal is to keep cheer accessible to everyone. 

Limited scholarships or fee assistance are available-please ask if needed. 

• Payments are due monthly. Late fees will be added if payment is received after the 10th of the month. 

• Payments can be made by cash or check. Any check returned for 

insufficient funds will be an additional fee of $75 and any acquired bank fees and check writing privileges will be suspended until further notice.
